Case managers at Rural Medical Services are part of your healthcare team. They work in conjunction with providers, nurses, care coordinators, and community organizations to support individual and family comprehensive health needs.
Case managers act as the “tour guides” to resources in our community that assist our patients in maintaining optimal health. They help patients achieve wellness and independence through advocacy, communication, education, identification of service resources, and service coordination.
Case managers work hard to promote patients’ safety, quality of care, and affordable outcomes. At Rural Medical Services we value our patients; our case managers, working as a team with our patients, will find the right plan and the most suitable resource to promote better care for a better life for each patient that is referred to case management.
